The episode opens with Huang Tianming offering Liu Ziyi money, hoping she'll refuse to operate on him. This unusual request sparks a conversation, but it's cut short when Liu Ziyi gets a fishbone stuck in her throat during dinner. Qin Wenbin, who happens to be on duty and at the same table, steps in to help. Their close proximity sparks some misinterpretations from onlookers, who initially mistake the situation for a romantic moment. Qin Wenbin, ever the professional, suggests they head back to the hospital to remove the fishbone, leaving the rest of the dinner party, including Wang Gaosheng, to finish their meal.

Family matters take center stage as Liu Ziyi's father visits the hospital with Aunt Jiang, who is admitted for a check-up, on Qin Wenbin suggestion. Liu's father brings two small boxes of cherries, intending to see his daughter. A coincidental elevator ride brings Liu Ziyi, her father, and Qin Wenbin together. Qin Wenbin encourages Liu's father to visit Aunt Jiang, and Liu Ziyi, sensing the awkwardness of a visit empty handed, gives one box of cherries to her father.

The past returns to haunt Liu Ziyi when Tang Mao, a patient who previously attacked her, is brought back to the hospital with a recurring illness. His brother insists that Liu Ziyi treat him. Understandably, Liu Ziyi refuses. This leads to a confrontation in the Director's office, where Doctor Cheng accuses Director Yin of favoritism, noting the difference in how Tang Mao and Huang Tianming's cases are assigned. Liu Ziyi remains firm in her refusal to treat Tang Mao. Doctor Cheng also states the case goes beyond his abilities. Director Yin, caught between the two strong-willed doctors, decides to take on Tang Mao's surgery himself. Liu Ziyi is then pulled aside by the director to discuss Huang Tianming, who has decided to forego treatment and move to a hospice. Liu Ziyi believes Huang Tianming still has a chance, but the director emphasizes the importance of respecting the patient's decision.

Concerned for Liu Ziyi's safety after Tang Mao's reappearance, Wang Gaosheng and the others rally around her. Wang Gaosheng provides her with pepper spray, and the group arranges to take turns escorting her home. While Liu Ziyi appreciates their concern, she assures them she'll be careful. Gu Shiyi, however, remains worried and vows to keep a close eye on her.

During a marathon club training session, Tang Mao suffers a sudden medical crisis. With other doctors unavailable, Zuo Feng contacts Liu Ziyi, who initially refuses to operate. However, her sense of duty as a doctor prevails, and she rushes back to perform a successful nine-hour surgery. After the surgery, Liu Ziyi learns of Huang Tianming's impending discharge and makes a last-ditch effort to convince him to continue treatment. She shares her experience with Tang Mao's surgery, emphasizing the importance of fighting through life's ups and downs. Huang Tianming listens, and although he still chooses to leave, Liu Ziyi believes he will eventually return.

The episode concludes with a tragic case. A patient named Xu Ming is brought in after a fall, requiring immediate surgery. However, delays arise when Xu Ming's aunt, a former doctor, demands a diagnosis report before consenting to the operation. The delay proves fatal, and despite the eventual surgery, Xu Ming doesn't survive.
