In this episode of "Good Will Society," the drama intensifies both in and out of the operating room. Guan Yifan, caught in a compromising situation, denies any infidelity to his wife, Liu Ziyi. He blames his odd behavior on overdoing it with health supplements, claiming it's all in preparation for starting a family. Ziyi, however, isn't buying his story, especially with her father-in-law, Guan Yifan's father, now facing health issues. She demands Guan Yifan arrange his father's transfer to another hospital, postponing any further discussion about their marital issues until his father recovers.
Meanwhile, cardiothoracic surgeon Qin Wenbin faces a challenging case: a young patient named Gao Nanxing with an external heart. Nanxing's strained relationship with his mother leads him to refuse treatment. In an attempt to persuade Dr. Qin to take extra care of her son, Nanxing's mother tries to discreetly offer him a bribe. This awkward exchange is witnessed by Liu Ziyi, who tactfully leaves the scene. Qin Wenbin, maintaining his integrity, refuses the bribe, determined to find a way to convince Nanxing to accept treatment. Ziyi's visit to Qin Wenbin was initially to inquire about her father-in-law's condition, but the encounter leaves her with a glimpse into the ethical dilemmas doctors sometimes face.
Wang Gaosheng, another doctor at the hospital, faces disappointment after failing his professional qualification exam. Adding insult to injury, a colleague who deliberately antagonizes him passes the exam. Wang seeks recourse from the hospital's vice president, only to be tasked with organizing a marathon. Despite his low spirits, Wang dutifully starts recruiting for a five-person marathon club. He approaches Qin Wenbin during lunch, and they eye Liu Ziyi, a former hospital track champion, and Gu Shiyi, a constantly-on-the-move emergency room doctor, as potential members. Ziyi, spotting them whispering, mistakenly assumes they're gossiping about her and Guan Yifan's marital problems.
Back in her department, Liu Ziyi's colleagues surprise her with wedding gifts, but she struggles to express joy, unsure how to navigate the situation given her crumbling marriage. Guan Yifan's grandmother visits, bringing a charm from a spiritual master, hoping to place it by her son's bedside. However, ICU rules prevent this. Ziyi, called to intervene, explains the situation to her grandmother-in-law, but the grandmother is appeased when Qin Wenbin surprisingly agrees to take the charm in. During a visit with Guan Yifan, his grandmother expresses her dissatisfaction with Ziyi, criticizing her perceived lack of care for the family. Guan Yifan, influenced by his grandmother's words, begins to believe Ziyi is neglecting him.
Gu Shiyi, all dressed up for a date, is called to handle an emergency, ending up covered in a patient's vomit. Jiang Yu, noticing her predicament, sends a nurse with a change of clothes. Gu Shiyi, refreshed and beautifully made-up, heads out for her date. A parking lot encounter between Qin Wenbin and Liu Ziyi leads to them exchanging WeChat contacts, after Qin intentionally parked too close to Ziyi's car. Ziyi, annoyed but restrained, retaliates by bumping his car mirror while driving off.
The episode concludes with Qin Wenbin meeting his wife, Pan Yue. Their interaction is formal and distant, highlighting their strained relationship. Pan Yue ultimately proposes a divorce. Liu Ziyi, working late, finalizes a surgical plan for a patient named Xiaoxiao, acknowledging the high risk involved. Xiaoxiao's father, desperate for a chance at treatment, signs the consent form.
